Once we confirm the payment we will make a package card and hold onto it until the patient's next visit. Please bring the payment confirmation and your ID when you pick up the package card.

**For our Senior (Age 60+), Student, and FCIM Student Family Members, we offer an 18% discount to this treatment package. To redeem this discount, just use the code FXAZEIM at check out. You must bring a valid government issued ID (Student ID) when you redeem your treatment package card.

If you are already in possession of a FCIM treatment package card, we can also reload it for you at your next visit.
